This project follows up on the Polish Hospital Feasibility Study. It will support the costs of a Home Care Manager. The Home Care Manager will look to address the short fall of home care on wards and will strategically develop a plan to identify new ways to recruit local actors to become Health Care Assistants. The Home Care manager will look at innovative ways to deliver home care that works well for clients the employees. Project area covers a large part of south Wrexham, encompassing 4 electoral wards and 12 Community Councils.
