Project Description:

The PIP (Prevention, Integration, Partnership) project aimed to develop and implement a total model of care. The Project was based in Solva, with PIP planning to benefit an increasing number of people in the community via the preventive healthcare programme and the development of activities aimed at extending the numbers of people taking part.

Project Outcomes:

  • Delivered help and support to Solva residents and supported their families via a community based care delivery system.
  • Developed a directory of local services and trusted traders
  • Developed IT support sessions and skills teaching for the elderly and has provided training and IT advice as well as advice and support to prevent and reduce the risk of cybercrime and fraud in partnership with the Police.
  • In partnership with other organizations engaged with and facilitated activities to improve well-being within the community including environmental and biodiversity initiatives.

Lessons Learned:

The project has demonstrated the complexity of NHS and Social Care provision and the lack of an integrated approach with shared aims, objectives and budgets. Care packages are difficult to deliver in rural areas due to a shortage of carers, long journey times and poor rates of pay. Information technology is not the solution to all problems; lack of connectivity and expensive equipment in the elderly population and in deprived areas is a problem.
