Enhanced Day Care Provision

Local authorities throughout Wales are facing significant reduction in resources whilst the demand for their services are increasing, therefore the LAs are increasingly collaborating with communities to provide services on their behalf. This particular model will look at enhancing the current day care provision available for individuals with low level care needs on Anglesey. As noted above, the day care service at the moment is only available at residential homes, which is not appropriate for individuals with low level care needs. This particular pilot will combine Age Well Hwyliog Mns Activities with dual role personal service day care provider. The qualified day care provider will be able to check service users blood pressure, sugar levels, as well as help with mobility, general health checks and assistance with toileting or eating, on the day. Combining this with the range of activities offered by Age Well makes it the first project of its kind throughout Wales.

Monitoring - The group will monitor the number new users, user demographics, referrals to GPs (increasing and decreasing) as well monitor referrals to other services such as physiotherapy, chiropody etc. This Pilot will look to support 10 individuals, over a period of 1 day per week for 6 months in the Amlwch catchment area. Area - Amlwch was identified as the best location for this pilot as it has been identified as a by the Local Authority as a target area within their Strategy for Older People. Amlwch is also known as an area of deprivation, has a higher number of recorded chronic illnesses and has an aging population. Promotion / Information Sharing The project will aim to work with 10 individuals therefore a small number of flyers will be created for the social services department, where most of the referrals will come from. If the pilot is successful it is likely to attract attention from other communities and the media, therefore the lessons learnt will be shared via social media, focus groups, IOACC and the partners websites.
